Through CHCA Project ECHO, we envision a future where all patients receive high-quality care, no matter where they live. Our innovative learning approach strengthens provider competencies by building essential skills, knowledge, and attitudes through education, best practices, and collaborative networks. Our goal is to improve care quality, strengthen clinical practice, support confidence and collaboration, and ultimately deliver better outcomes for patients and their families.

Home Care Project ECHO delivers practical solutions

We are closing the knowledge gap in home care, primary care, and community-based services by providing easy access to expertise, experience, and evidence-based practices. Through Project ECHO, we:

  • Offer accessible education that overcomes geographic, financial, and time barriers.
  • Foster communication and collaboration among multidisciplinary care providers.
  • Promote continuous learning through evolving clinical practices and practical tools.

ECHO Streams

Each ECHO Stream addresses priority areas in home care, primary care, and community-based services. Modelled after the global Project ECHO movement, we connect you with specialists, experts, leading practices and practical tools. Here’s what you can expect:

  • TeleECHO Learning Sessions – One-hour virtual sessions featuring clinical experts and providers from across Canada.
  • Case-Based Learning – Real-life case studies that apply concepts and approaches in practical settings.
  • Best Practice Protocols – Evidence-informed practices and protocols tailored to each topic and theme.
  • Continuing Education – Access to resources and tools through the online ECHO Hub to support lifelong learning.

The Project ECHO Home and Community Care Learning Hub offers a variety of resources for healthcare providers delivering home- and community-based care. Through the hub, you can access:

  • On-demand TeleECHO session recordings that feature expert presentations and case-based discussions.
  • Presentation slides from subject matter experts
  • Evidence-based resources, including articles and reports supporting clinical practices.
  • 10-minute micro-learning courses on key learning content for a selection of TeleECHO topics.
