The Way Forward: An Integrated Palliative Approach to Care

The Way Forward: An Integrated Palliative Approach to Care sets a national framework for a high-quality care palliative care across all care settings, including home care, long-term care and primary care. This seminal framework reflects input from federal, provincial and territorial governments, health authorities, clinicians and service providers who identified best practices, tools and resources. The CHCA was an active participant serving on the project advisory group and engaging community and home-based care stakeholders in the ongoing work.   Visit website   |   Download Framework

A Palliative Approach to Care: Consultations with Caregivers

As part of the CHCA work on The Way Forward: An Integrated Palliative Approach to Care initiative we reached out to caregivers of individuals with life-limiting illnesses to gain insights into their unique needs and challenges. This report reflects our findings.

The Pan-Canadian Gold Standard for Palliative Home Care

The CHCA in partners with the Canadian Hospice Palliative Care Association has defined the “gold standard” for each of the four home care services that are provided through the publicly funded health care systems for individuals with palliative needs. The services include case management, nursing, palliative-specific pharmaceuticals and personal care.
