Each province and territory have their own publicly funded health authority, service, or home care program where individuals can self-refer or request home and community care services or supports. The federal government delivers home care to First Nations on-reserve and Inuit in designated communities, members of the armed forces and the RCMP, and eligible veterans. It is important to note that eligibility, accessibility, and the delivery of services vary across jurisdictions.
Caregivers are central to home and community care. While providing care can be rewarding, it can also be challenging. Many caregivers manage care responsibilities while balancing personal, professional, and family obligations. The resources below are intended to support both people receiving care and those who care for them by highlighting publicly funded home and community care services, as well as caregiver-specific supports across Canada.
This list is a starting point to help individuals, caregivers, families, and those who support them explore available services and connect with supports in their area.
